Memorial Garden at Chautauqua Park
Chautauqua Park, located in south Lakeside on the corner of Poplar Avenue and Sixth Street, is a sacred space dedicated to celebrating the circle of life. A portion of land will be used for The Memorial Garden at Chautauqua Park.
Vision
Within The Memorial Garden at Chautauqua Park, the cremated remains of loved ones will be placed in the ground, within a biodegradable container, to become part of the soil, giving life to the garden’s many perennial plants and trees.
This memorial garden serves as both a final resting place for Lakesiders, as well as a place where the living can reflect and connect spiritually, loved ones can be remembered, special occasions can be celebrated and the beauty of nature can be embraced. It is a place of discovery, spiritual enrichment and reflection.
Fees & Placement
Placement of Ashes in The Memorial Garden at Chautauqua Park is open to all. The placement fee for ashes helps sustain and maintain the beautiful garden.
For adults, the placement fee is $1,500, plus an optional $500 if the family wishes to list their loved one on an inscribed feature. For children under age 18, the placement fee is $500, plus an optional $500 if the family wishes to list their loved one on an inscribed feature. The optional $500 is paid at the time the ashes are placed. This fee cannot be collected earlier.
To arrange placement of a deceased love one, download and return the Placement of Ashes Agreement and Placement of Ashes Service and Preparation Form to: Lakeside Chautauqua, Attn: The Memorial Garden at Chautauqua Park, 236 Walnut Ave., Lakeside, OH 43440. Fees and arrangements can be pre-planned at any time by completing the forms listed above and by paying the non-refundable fee. All checks for fees may be made payable to The Memorial Garden at Chautauqua Park, LLC.
When the application is received, along with the fee, a Lakeside Chautauqua representative will return a Placement of Ashes Certificate for the estate and family records. A copy of this certificate is also recorded at Lakeside Chautauqua. Services & Receptions
Lakeside Chautauqua offers memorial services. These shall be performed under the leadership of the Lakeside Chautauqua Director of Religious Life & Church Outreach.
A family may also have other clergy lead the service; however, a member of The Memorial Garden at Chautauqua Park Committee will need to be present for official document signing. To coordinate a memorial service after the placement fee has been paid, please download and complete the Placement of Ashes Agreement and Placement of Ashes Service and Preparation form.
Management & Care
A core group of volunteers and a third-party professional landscaping team will carefully and lovingly take care of the beautiful garden. The space will be managed by The Memorial Garden at Chautauqua Park Committee, which is designated by the Lakeside Chautauqua Board of Directors. The committee is responsible for oversight of the memorial garden, implementation of policies and maintenance.
Construction and maintenance of the garden are financed by placement fees, fundraising and the endowment trust. The Memorial Garden at Chautauqua Park endowment trust of $50,000 has been established to ensure long-term maintenance. Gifts to the memorial garden are welcome at any time.
